Thank you for your interest in The Golden Qi Foundation!  We are a 501(c)3 non profit organization that provides free massage therapy and support to senior citizens aged sixty-five years and older in Louisiana and Georgia. If you are a senior citizen aged sixty-five years or older living in the Atlanta metro area in Georgia or New Orleans or Baton Rouge, Louisiana, then complete the application form below to be considered to receive free massages. In addition to completing the form below, you must attach a picture of your valid driver's license or US Identification Card and proof of clearance to receive table massage therapy for sixty minutes from your Healthcare Provider on your Healthcare Provider's letterhead .
